Background Check Authorization and Disclosure Forms

***THE FOLLOWING ARE EXEMPT FROM BACKGROUND CHECK WITH PROPER CREDENTIALS

• Active Duty Military
• Active Commissioned Law Enforcement
• Active EMT with Employment
• Active Nevada CCW Holders 

Everyone else must go through the background check process, thank you for helping keep our community safe. 

Guardian Academy LLC (“the Company”) may obtain a “consumer report” about you from a consumer reporting agency for membership and/or participation purposes. A “consumer report,” for purposes of this disclosure, will be limited to a background screening report containing information regarding your criminal history only.

This information will be used solely to determine your eligibility for membership, participation in training programs, and access to Company facilities. The report may bear upon your character and general reputation as it relates to maintaining a safe and secure training environment.

The Company does not obtain or review information related to credit history, driving records, or other non-criminal background information for membership purposes

One-time fee is $25, subject to change based on background check provider.

Payments can be made online or with cash in person.

By submitting your Authorization for Background Check, you confirm you have received the Background Disclosure information.
